I am a U.S. citizen or lawfully admitted permanent resident.

This contribution is made from my own funds, and funds are not being provided to me by another person or entity for the purpose of making this contribution.

I am at least eighteen years old.

I am making this contribution with my own personal account and not with a corporate or business account or an account issued to another person.

Please include your name, address, phone number, occupation, and employer. If you are retired, you must list your occupation as "retired." You do not need to list an employer. This information will only be used for mandatory government reporting and will not be provided to any business or political entities. ​​

Contributions or gifts are not tax deductible. The maximum amount an individual may contribute is $470 per election. Your contribution (up to $470) will be designated for the primary election. The next $470 will be designated for the general election.
